Practice Update

We are pleased to announce that Southeast Perinatal Associates will be joining Pediatrix Medical Group, Inc. The transition is currently estimated to occur on August 31, 2025, but may be extended to a later date. Any updates to the transition date will be posted on our website, and a corresponding physical notice will be posted at our office. Our practice will remain at its current location, and the same clinical care team will continue to provide the same high-quality care you have come to trust.

During this transition, if you would like a copy of your medical or billing records, please contact the office at the location where you have received care, as listed below. After the transition date, if you would like a copy of your medical or billing records, please contact a patient liaison by calling (800) 207-8484 or faxing (833) 493-2772.

As always, you have the right to choose your healthcare provider. If you choose a provider outside of Southeast Perinatal Associates, you may request a transfer of your medical records to your new provider. To facilitate a seamless transition of your medical records, we ask that you complete the Authorization for Release of Information. Through the transition date, please submit the completed form by fax or mail to the office where you received care, as listed below. To transfer records after the transition date, please call (800) 207-8484 to work with a patient liaison or fax your completed form to (833) 493-2772.
